If your team is interested in attending EMSA City Finals for the current season, the SPDSA will need the following application filled out.
By filling out this form you acknowledge that if your team qualifies for EMSA City Finals you must attend the competition. You as an SPDSA team cannot qualify for provincials through City Finals, SPDSA has its own birth into provincials and a separate application form needs to be filled out to declare for provincials.
